A Clinical Study on Liver Abscess


Abstract
This study is a clinical analysis of 152 patients with liver abscess admitted to the Dept. of Surgery, Pusan National University Hospital during the period of 10 years from January, 1983 to December, 1992 with review of the literature
@ES The results obtained were summerized as follows:
@EN 1) The peak age incidence was 5th decade and male to female ratio was 2.2:1, O all 152 cases with liver abscess, 105 cases(69.1%) were pyogenic, 42 casese (27.6%) were abebic, and 5 cases(3.3%) were abscess.
2) The liver abscess was more often involved in right lobe(55.3%) than in left lobe (30.3%). The most common symptoms and signs were abdominal pain(81.6%), fever and chilling(74.3%) and abdominal tenderness(75.7%).
3) Leukocytosis was presented in 135 cases(88.8%) and serum akaline phosphatase was increased in 79 cases(52.1%). In the pyogenic abscess, the most common causative organism was Escherchia coli(38.7%), All the cases with amoebic abscess(47
cases)
were
confirmed by smear of abscess contents.
4) Operative drainage was done in 122 cases(80.3%) of all treated patient and percutaneous aspiration with drainage was in 27 cases(17.7%). Among the aspiration group, reoperation was needed in 9 cases(5.9%). Additionaly laparoscopic closed
drainage
was done in 3 cases(2.0%) of total patients. In the result of treatment, defervescence period was 4.45¡¾2.02 days, 6.39¡¾3.82 days and 3.00¡¾0 days in operation, aspiration and laparoscopic closed drainge respectively. And leukocytosis was
normalized on
5.67 2.89 days, 9.01¡¾3.57 days and 4.67¡¾0.47 days respectively(p<0.01 in both).
5) Complications occured in 23 cases(15.1%) with operation and 5 cases(3.3%) with aspiration groups. Death occured in 7 cases(4.6%) with operation and 1 cases(0.7%) with aspiration groups and most common cause of death was adult respiratory
distress
syndrome
